Background:To date, Nolvadex remains the first selective estrogen receptor modulator (SERM).
Objectives:To determine the efficacy of Nolvadex in premenopausal women with early breast cancer. The objective was to compare efficacy between tamoxifen (Nolvadex) and placebo. This was a double-blind randomized placebo-controlled trial. Participants were 40 postmenopausal women with hormone receptor-positive early breast cancer who were being treated with Nolvadex for five years. Primary end point was the overall survival (LOS) in the adjuvant and disease-free status. Secondary end points included recurrence-free survival (RFS) and overall survival (OS).
Methods:Randomization was performed by an Independent Ethics Committee (IPC) at our Cancer Centre (NCT01127545). The study was conducted in accordance with the principles of the Declaration of Helsinki. The protocol was approved by the Ethics Committee and by the International Ethics Committee of the Medical School. Exact results were reported to the IPC and were based on an evaluation of the protocol and the patients' characteristics. All patients gave written informed consent.
Results:The mean age was 58.3 years (range: 20-85 years) and 55.1% were women. Most (80%) were white. Most (92.4%) had breast cancer or were at a high risk of breast cancer. In the tamoxifen group, Nolvadex was significantly more effective in the RFS (17.9% vs. 8.6%) and OS (12.6% vs. 4.9%). In the placebo group, the median Nolvadex dose was 1.9 mg/day. Nolvadex was more effective in the RFS (19.8% vs. 5.7%), but not in the OS (4.9% vs. 1.9%). The median Nolvadex dose was 4.6 mg/day. The median RFS was not significantly different between the two groups (7.2% vs. 6.9%).
Conclusions:Nolvadex is a highly effective and well tolerated SERM. While a high Nolvadex dose may be useful in the adjuvant treatment of early breast cancer in women with early breast cancer, it should be taken seriously for women with early breast cancer who are at high risk of breast cancer.
